I give Camp Ton-A-Wandah permission to use in its catalogue or on its website any camp picture in which the likeness of my daughter appears. Also, I hereby give permission to the physician selected by the Camp Administrator to hospitalize, secure proper treatment for, and to order injections, anesthesia and surgery as needed for my child named above.
I understand that there are a number of inherent risks involved in summer camp activities. I, my spouse, my child, and my heirs, successors, assigns and personal representatives, hereby release and discharge Camp Ton-A-Wandah, their representative agents, employees, officers, directors, volunteers and successors and assigns (hereinafter referred to individually or collectively as "Releasees") from any liability or causes of action whatever rising from, or on account of, property damage, economic loss, personal injury or death, related to my child's participation in the camp, including, without limitation, any liability or causes of action based on, asserting, or cause by the negligence of Releasees or other persons.
There will be no reduction of fees for late arrivals or early withdrawals, including homesickness. Only in cases of early withdrawal due to serious illness will a refund be considered. The right is reserved to dismiss any girl whose presence, in the Director’s judgment, is detrimental to the best interest of the camp, and in such case there will be no reduction in reservation fee. If cancellation is made on or before February 1, $200 of the $400 reservation will be refunded upon written notification to the camp office. After this date, the entire reservation fee will be forfeited. Cancellation on or after the 14th day before opening day of camp will result in a forfeit of the entire tuition.
